Smoking and Vaping: What They Are, How They Affect Your Health and Your Gut
Smoking and vaping are two different ways of inhaling substances into the lungs, but both can expose the body to potentially harmful chemicals. Smoking has been linked to a wide range of serious health conditions for decades, while vaping is a relatively newer phenomenon and scientists are still investigating its long-term effects.
With millions of people smoking or vaping in the UK, understanding what these products contain and how they may affect the lungs, heart, brain and gut is increasingly important.
What Is Smoking?
Smoking involves burning tobacco and inhaling the resulting smoke. Tobacco smoke contains thousands of chemicals, including nicotine, carbon monoxide and numerous toxic substances. Many of these chemicals are produced or released during combustion.
Nicotine is the main addictive substance in tobacco. It stimulates the brain's reward pathways, making smoking highly addictive and often making it difficult for people to stop.
However, nicotine is not the only concern. The combustion of tobacco produces a complex mixture of chemicals that can damage cells and tissues throughout the body.
Smoking is a major risk factor for serious diseases, including lung cancer, chronic obstructive pulmonary disease (COPD), heart disease and stroke. It can also damage blood vessels, weaken the immune system and affect the body's ability to support.
What Is Vaping?
Vaping involves using an electronic cigarette or vape device to heat a liquid, commonly called e-liquid or vape juice, to create an aerosol that is inhaled.
E-liquids commonly contain nicotine, flavourings, propylene glycol and vegetable glycerine, although the exact ingredients vary between products.
Unlike traditional cigarettes, vaping does not involve burning tobacco. This means that vaping generally exposes users to fewer toxic chemicals than smoking cigarettes. For adult smokers who switch completely from smoking to vaping, this is one reason vaping is generally considered less harmful than continuing to smoke.
However, "less harmful" does not mean harmless.
Vaping can still expose the lungs and body to nicotine and other chemicals. The long-term health effects of inhaling vape aerosol over many years are not yet fully understood.
For people who do not smoke, starting to vape offers no established health benefit and may introduce an unnecessary source of nicotine exposure.
How Does Smoking Affect Your Health?
The health effects of smoking are extensive.
Smoking damages the respiratory system and can cause long-term inflammation and structural damage in the lungs. It is a leading cause of COPD and is strongly associated with lung cancer.
The cardiovascular system is also affected. Smoking damages blood vessels, increases the risk of blood clots and contributes to heart disease and stroke.
Smoking can also affect fertility and reproductive health, increase the risk of complications during pregnancy and impair wound supporting recovery.
Because nicotine is addictive, dependence can make stopping difficult. Withdrawal symptoms may include irritability, anxiety, low mood, difficulty concentrating and strong cravings.
The good news is that stopping smoking can provide health benefits at any age. The body begins to recover after quitting, with many health risks reducing over time.
How Does Vaping Affect Your Health?
Vaping is generally considered less harmful than smoking because it does not involve tobacco combustion. However, research continues into the potential long-term effects of vaping.
Nicotine-containing vapes can cause dependence and may increase heart rate and blood pressure. Nicotine exposure can be particularly concerning for young people because the brain continues developing into early adulthood.
Vaping may also irritate the respiratory system. Some users experience coughing, throat irritation, headaches or other symptoms.
The composition of vape aerosol can vary considerably depending on the device, liquid and temperature. Some aerosols may contain substances that can irritate or damage lung tissue.
For current smokers, completely switching to vaping is likely to reduce exposure to many of the harmful chemicals found in cigarette smoke. However, people who both smoke and vape may not experience the same level of risk reduction as those who stop smoking completely.
Smoking, Vaping and the Gut
An emerging area of research is the relationship between smoking, vaping and the *gut microbiome*.
The gut microbiome is made up of trillions of bacteria and other microorganisms living throughout the digestive system. These microbes play important roles in digestion, immune function and metabolism.
Smoking has been associated with changes in the composition of the gut microbiome. Research suggests that tobacco exposure may contribute to alterations in microbial diversity and balance, although the exact mechanisms and long-term consequences are still being investigated.
Smoking is also linked to inflammatory processes throughout the body. Because the gut and immune system are closely connected, researchers are interested in whether smoking-related changes to the microbiome may contribute to inflammation and disease.
Vaping and gut health are less well understood because vaping is relatively new. Early research suggests that exposure to nicotine and other components of vape aerosols could potentially influence inflammation and microbial processes, but there is currently much less evidence than there is for smoking.
It would therefore be premature to claim that vaping definitely causes gut dysbiosis or specific digestive diseases. However, the possibility of effects on the gut microbiome is an area that warrants further research.

Vaping is generally considered less harmful than smoking for adults who smoke and switch completely, but it is not risk-free. The long-term effects of vaping are still being studied, particularly regarding the lungs, cardiovascular system and gut health.
For people who do not smoke, there is no health reason to start vaping. For smokers, stopping completely remains the best option for health. However, switching completely from cigarettes to vaping may reduce exposure to many harmful chemicals compared with continuing to smoke.
As research develops, we are likely to learn more about how inhaled substances influence not only the lungs but the wider body — including the complex ecosystem of microorganisms living inside our gut.
