The condition is often described as *delayed gastric emptying*, meaning that food moves from the stomach into the small intestine more slowly than it should. In some cases, the stomach may empty very slowly or stop emptying normally, despite there being no physical blockage preventing food from moving through the digestive system.

What is gastroparesis?

Normally, after we eat, the muscles in the stomach contract to break down food and gradually move it into the small intestine. This is an important part of digestion, allowing the body to continue processing food and absorbing nutrients.

With gastroparesis, the nerves or muscles involved in this process may not work properly. As a result, food can remain in the stomach for longer than expected.

This can lead to symptoms that begin or become more noticeable after eating.

Common symptoms may include:

  • Feeling full very quickly when eating

  • Feeling uncomfortably full for a long time after a meal

  • Nausea

  • Vomiting

  • Bloating

  • Upper abdominal pain or discomfort

  • Heartburn

  • Reduced appetite

  • Unintentional weight loss in some people

The severity of symptoms can fluctuate, with some people experiencing periods when symptoms are better and others when they become more difficult to manage.

What causes gastroparesis?

There is not always one clear cause.

Diabetes is one of the most common known causes. Over time, high blood glucose levels can damage nerves that help control the stomach, including the vagus nerve, which plays an important role in the movement of food through the digestive system.

Gastroparesis can also develop following certain types of surgery if nerves involved in stomach function are affected. Other possible causes or associated conditions include some neurological disorders, hypothyroidism and certain autoimmune conditions.

In some cases, no underlying cause can be identified. This is sometimes referred to as *idiopathic gastroparesis*.

The condition is complex, and more research is needed to fully understand why it develops in some people and not others.

Gastroparesis and nutrition

One of the challenges of living with gastroparesis is that foods that are generally considered healthy may not always be easy for someone with delayed gastric emptying to tolerate.

For example, higher-fibre foods can be difficult for some people to digest, while high-fat meals may slow stomach emptying further. This means that dietary advice for someone with gastroparesis often needs to be individualised.

Healthcare professionals may recommend eating smaller meals more frequently rather than three large meals. Some people may also find softer or well-cooked foods easier to manage, and in more severe cases, liquid or blended meals may be recommended.

The NHS advises that people with gastroparesis may be referred to a dietitian to help ensure they receive adequate nutrition while managing their symptoms.

This is particularly important because ongoing nausea, vomiting or difficulty eating can increase the risk of dehydration, weight loss and nutritional deficiencies.

Is there a connection between gastroparesis and gut health?

The term "gut health" is often used broadly to describe the health of the digestive system, including the gut microbiome — the community of microorganisms living within the digestive tract.

There is growing interest in the relationship between the gut microbiome, digestion, the immune system and overall health. However, it is important to distinguish between what is currently known and what is still being researched.

Gastroparesis is primarily associated with problems involving the nerves and muscles that control stomach movement. At present, there is *not enough evidence to say that an unhealthy gut microbiome causes gastroparesis*, nor is there evidence that improving gut health alone can support the condition.

However, digestive health and nutrition remain important considerations for anyone living with gastroparesis. Because food may remain in the stomach for longer, and because symptoms can make eating difficult, maintaining adequate nutrition and hydration can sometimes require specialist support.

For this reason, anyone with diagnosed or suspected gastroparesis should be cautious about following restrictive diets or taking supplements without professional guidance. What may be helpful for one person's digestive health could potentially make symptoms worse for someone with delayed gastric emptying.

Supporting your health with gastroparesis

Managing gastroparesis usually involves a combination of approaches based on the individual's symptoms, underlying cause and nutritional needs.

This may include dietary changes, medication to help with nausea or stomach emptying, and management approaches of an underlying condition such as diabetes. In more severe cases, additional medical management approaches may be considered.

If you have gastroparesis, keeping a record of your symptoms and how you respond to different foods may help you and your healthcare team identify patterns. However, it is important not to help reduce large numbers of foods without professional advice, particularly if you are already struggling to maintain your weight or nutritional intake.

Gentle movement, such as walking after eating, may be recommended for some people, while avoiding lying down immediately after meals may also help with certain digestive symptoms. Individual advice from your healthcare team should always take priority.

When should you seek medical advice?

Persistent symptoms such as feeling full very quickly, repeated nausea or vomiting, ongoing abdominal pain, or unexplained weight loss should not simply be dismissed as "poor digestion".

These symptoms can have many possible causes, and it is important to receive an appropriate medical assessment.

Seek urgent medical attention if you experience severe or sudden abdominal pain, vomit blood or material that looks like coffee grounds, or have signs of significant dehydration or severe weakness.

Living with gastroparesis

Gastroparesis can be a challenging condition, but support and management approaches are available. The right approach will depend on the individual, and managing the condition is often about finding a balance between symptom control, adequate nutrition and quality of life.

While research into the digestive system and gut microbiome continues to develop, there is still much to learn about the relationship between gut health and conditions affecting digestion.

For now, the most important step is to seek appropriate medical advice, understand your individual triggers and nutritional needs, and work with qualified healthcare professionals to develop a management plan that is right for you.
